Although DNA methylation plays a critical role in leukemia biology, its clinical application in acute myeloid leukemia (AML) has been limited. By analyzing DNA methylation data from 3,314 patients across 11 cohorts, this study introduces the Acute Leukemia Methylome Atlas, which accurately predicts AML subtypes and five-year survival using both genome-wide models and a targeted 38-CpG panel. A rapid specimen-to-result workflow using nanopore sequencing and machine learning demonstrated strong clinical concordance, highlighting a path toward faster, more affordable, and precise molecular diagnosis and prognosis despite challenges with rare karyotypes.
